Mandatory for all patients having a procedure at Atlanta Outpatient Surgery Center. At the home page, please click on the "Clinical History" tab to begin, complete each page until you reach the end. This should be completed less than 7 days prior to the procedure. If you do not have internet access, you may complete a copy of the clinical history which you received from Dr. Layne's office, and fax to 1-866-283-1886. If you have additional questions or need help completing the questionnaire, please call the Surgery Center at 404-252-3074 .
